1) I found this mushroom growing on the ground on SFU mountain.
2) The cap is planar with a fairly even cap margin. It looks smooth. There is no partial veil and it has adnate gills. The gills are even and crowded. The stipe is central, equal, and longitudinally striated.
3) I had to go use other resources besides the lab notes to figure out which mushroom this is. It looks somewhat like Hebeloma velutipes or Inocybe whitei. Molecular investigation would be able to confirm this or not.
